How to make appointment?

Please contact us by phone or email for an appointment.

What should I bring?

  • You are required to bring a valid referral (to Centre For Specialist Medicine & Geriatrics or direct to our specialist) from either your general practitioner or specialist in order to claim any Medicare rebate prior to review.
  • We aim to provide you earliest available appointment with your chosen Geriatrician or the first available specialist in the group.
  • Please bring your Medicare card, Health-care / Pension card, DVA card or Private health fund card.

In addition, please bring:
  • A list of your past medical conditions (including previous specialist letters and hospital discharge summaries).
  • A list of your current medications.
  • All previous brain scan (CT/MRI), X-rays, recent blood test and urine test results.

You are strongly encouraged to bring along a close family/ friend for the consultation.

How long will my appointment take?

Please allow up to 1 hour for your appointment. Please arrive 15 minutes before the appointment to complete the relevant paperwork.

How can I pay?

Please contact us for more information regarding our fee structure. We accept all major credit cards and EFTPOS. You will be entitled to a Medicare refund provided there is a referral letter from your GP or specialist.
